How to get your card **

1. Call MariTrust and speak with a representative. They will walk you through the scheduling of your appointment at our doctor’s office and answer all of your questions/concerns. The doctor’s visit is *$100, and the notary is an additional *$5.

2. After scheduling, attend your appointment with any pertinent medical records you may have. If the doctor agrees that you are a viable candidate for cannabis therapies, he will provide you with a recommendation.

3. Return to MariTrust and remit recommendation to a staff member to ensure proper completion/delivery. The state medical marijuana registry application fee is *$90.

4. You are now legally allowed to purchase, possess, and use (with some restrictions) medical marijuana. After completing MariTrust’s intake forms, you will be taken to the medicine room.**
